CONTACTING YOU, I WILL JUST VOID THIS APPLICATION. DOUBLE CHECK THIS BEFORE

The ONLY person that will be submitting this questionnaire is the person who is purchasing the puppy and where the puppy is going to live with. NO EXCEPTIONS. If I find out different, this will be breaking the contract and penalties will be paid to me by the person who broke the contract. I do not allow other people purchasing as gifts. Where the puppy lives IS the person who will submit this questionnaire, make the payment, and sign the contract. NO excuses at all, absolutely no exceptions, NONE whatsoever! If a person cannot follow my rules, then please do not submit this questionnaire.
